We are located in the Medical Office Building, which is on Main Street on the border of Layton and Kaysville. Most insurances accepted. If you do not see your insurance, call our office to verify your plan.

It is not necessary to obtain a referral for treatment for most insurances. Call our office if you have any questions. We see regular office appointments as well as emergency, trauma, Work Comp and urgent care patients. For after hours and weekends, trauma, acute fractures and dislocations may call the office number and the phone service may reach me for immediate treatment. Dr. Warby is also on call at Davis Hospital Emergency Room and treats many trauma cases through the ER.
